Immigration Medical Examination Services
The immigration medical examination is conducted in accordance with the United States Homeland Security -U.S. Citizenship and Immigration Services (USCIS) which entails a review of your medical history, physical examination and immunizations. Testing for communicable diseases and immunizations are performed depending on age, as required by the Centers for Disease Control and Prevention (CDC).
What to bring to your First Appointment?
- Government issued identification - Drivers License or Passport
- Proof of childhood and/or adult vaccinations
- Proof of Covid-19 vaccination
- I-693 Form (USCIS - click link below for form)
